LVI’s “Dream Smile” Contest Rules and Regulations |

Contest Ends on September 30, 2010 at 11:59PM.
How to Enter: Visit www.lvismile.com and find an LVI Dentist in your area. Schedule and complete a consultation with the doctor (All consultation costs, if any, will be incurred by entrant). All entrants must have completed their "entry consultation" by 11:59PM on September 30, 2010. No incomplete or forged entries will be accepted.
Selection of Winner: Winning entrant will be selected at random by October 15, 2010 from all eligible entries received. The winning entrant will be notified no later than January 15th, 2010. Winner agrees that they will not disclose that they have won until after the official announcement of the winner on January 15th, 2010. Winner will be notified by e-mail.
Prize: Winner will receive up to $20,000 credit toward an LVI Smile provided by their local LVI Dentist where they received their entry consultation. No substitution, assignment, or transfer of prize is permitted. Sponsor is only responsible for prize delivery; not responsible for prize utility, quality, or otherwise. Taxes and fees, if any, are the sole responsibility of the prize winner.
Eligibility: Contest is open to all individuals that are ages 18 and over on or after August 3, 2009. Entrants must be legal residents of the fifty United States of America or Canada. Employees (including immediate family and those in the same household), affiliates, agents, advertising agencies & promotion agencies of Sponsor are deemed ineligible.
Verification of Winner and Delivery of Prize: Winner will be required to sign an affidavit of eligibility, a liability/claims release, and a publicity release. Prize may be forfeited if affidavit is not signed and returned within 3 weeks of notification to winner or if prize is not used within one calendar year of date of notification. If prize is won by a minor, it will be awarded, at Sponsor’s sole discretion, in the name of the minor’s parent or legal guardian. Winner agrees to permit Sponsor’s use of name, address, likeness, and/or prize information for promotional purposes within any medium without additional compensation to the extent permitted by law. Any such use with respect to minors is subject to prior approval from the minor’s parent or legal guardian.
Odds: Odds of winning depend on the number of entries received. Entries will not be limited, except as expressly provided above.
General: This contest is void where prohibited or restricted by law and subject to all applicable federal, state and local laws, rules, and regulations. The contest will be governed by the laws of the State of Washington, USA and any applicable U.S. Federal laws. Any and all legal actions or claims arising in connection with this contest must be brought in King County Superior Court, State of Washington, USA. Sponsor is not responsible for late, lost, stolen, damaged, garbled, incomplete, misaddressed, postage-due, or misdirected entries, mail or communications, for errors, omissions, interruptions, deletions, defects, or delays in operations or transmission of information, in each case whether arising by way of technical or other failures or malfunctions of computer hardware, software, communications devices, or transmission lines or data corruption, theft, destruction, unauthorized access to or alteration of entry materials, loss or otherwise. Receipt of any prize is conditioned upon compliance with any and all applicable federal, state and local laws, rules and regulations. Sponsor is not responsible for electronic
communications which are undeliverable as a result of any form of active or passive filtering of any kind, or insufficient space in entrant’s e-mail account to receive e-mail messages. Sponsor disclaims any liability for damage to any computer system resulting from participation in, or accessing or downloading information in connection with, this contest, and reserves the right, at its sole discretion, to modify, cancel, terminate or suspend the contest should any virus, bug, technical failures, unauthorized human intervention or other causes beyond Sponsor’s control corrupt or affect the administration, security, fairness or proper conduct of the contest. In the event of any such cancellation, termination or suspension, a notice will be posted and the winner will be selected from among all eligible entries received prior to such time. Sponsor reserves the right, at its sole discretion, to disqualify any entrant from this contest or any other promotion conducted now or in the future by Sponsor or any of its affiliates if his or her fraud or misconduct affects the integrity of the contest. By participating in this contest, each entrant agrees to warrant that s/he is eligible to participate in this Contest. By accepting prize, winner agrees to release Sponsor, its directors, employees, officers, and agents, including without limitation, its advertising and promotion agencies, from any and all liability, loss or damages arising from or in connection with the awarding, receipt, and/or use or misuse of prize or participation in any prize-related activities.
Caution: Any attempt by an entrant to deliberately damage any website or undermine the legitimate operation of this contest is a violation of criminal and civil laws. Should such an attempt be made, Sponsor reserves the right to seek damages from any such individual to the fullest extent permitted by law in any court of competent jurisdiction within the United States.
